Nurturing Remedies – Integrative Nutrition & Herbal Care

By participating in services provided by Nurturing Remedies, you acknowledge and agree to the following:

SCOPE OF PRACTICE
I understand that the services provided are educational and supportive in nature and are not intended to diagnose, treat, cure, or prevent disease. Nutritional and herbal recommendations are not a substitute for medical care.

PROFESSIONAL ROLE
I understand that Christine Kane is a Registered Dietitian and herbal practitioner providing integrative nutrition education, functional support, and lifestyle guidance. Services may include nutrition counseling, functional testing interpretation, herbal education and products, and wellness planning.

MEDICAL CARE
I acknowledge that I am responsible for consulting with my physician or other qualified healthcare providers regarding any medical condition, medication changes, or treatment decisions.

RISKS AND INDIVIDUAL RESULTS
I understand that responses to nutrition, supplements, herbs, and lifestyle changes vary by individual. No specific outcomes or results are guaranteed.

CLIENT RESPONSIBILITY
I understand that my progress depends on my participation, commitment, and follow-through.

CONFIDENTIAL AND PRIVACY
Personal health information is kept confidential in accordance with applicable privacy laws. Information is shared only with client consent or as required by law. Reasonable safeguards are used to protect personal and health information.

VOLUNTARY PARTICIPATION
I understand that I may discontinue services at any time.

SCHEDULING AND CANCELLATION POLICY
Appointments require at least 48 hours’ notice to cancel or reschedule. Late cancellations may be charged a fee of $95 or 50% of the session fee. Missed appointments without notice may be charged the full session fee. Emergency situations are handled with discretion.
